Termite Control Supervision, Recordkeeping and Customer Disclosure





In general, the category 7b registered technician must have a copy of the label on site with him during his applications. The following ten items must also be given to the technician by the certified category 7b supervisor and must be kept on file by the business for at least 5 (five) years:

  1. A copy of the contract, including date of treatment.

  2. A diagram/graph of the structure to be treated.

  3. Dimensions of the structure to be treated. Include depth-to-footer if soil liquids are used.

  4. Treatment specifications, if soil liquids are used.

  5. EPA registration number(s) of the termite control product(s) used.

  6. Dilution rates for all soil applied or wood applied liquid products used.

  7. Total volume of each soil applied or wood applied liquid product used.

  8. Name and registration number of the registered technician(s) performing the treatment, if applicable.

  9. Name and certification number of the Category 7b certified applicator performing the treatment or responsible for supervision of the registered technician(s).

  10. A copy of the Termite Control Customer Disclosure Form. This form is used to disclose to customers in writing any label directed termiticide application procedure that was not performed as part of the termite control treatment. It is required to be signed by the customer and kept as part of the five-year records.
